Waiver

I, the parent/guardian of the registered player, agree and understand that the coaches, coordinators and volunteers of the DALLUYON Basketball shall not be held responsible for any liability, injury or accident, during or travelling to and from events, games and/or practices. Without limiting the generality of the foregoing, I acknowledge that basketball is a physical game and that injuries may occur. I further grant permission to DALLUYON Basketball to use all personal information including pictures and images gathered for the purposes of DALLUYON Basketball including but not limited to, reporting to governing bodies/agencies and for the use in advertising and promotional materials.

DALLUYON Basketball has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you, your child(ren), or family members who visit our rented facility will not become infected with COVID-19. Further, attending DALLUYON Basketball could increase your risk, your child(ren)’s risk, and your family's risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren), my family, and I may be exposed to or infected by COVID-19 by attending DALLUYON Basketball and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at DALLUYON Basketball training programs, camps, team practices and tournaments may result from the actions, omissions, or negligence of myself and others, including, but not limited to, DALLUYON Basketball coaches, facility employees, volunteers, and program participants and their families.

Privacy Policy

Personal information collected from you is collected under the authority of S.32c of the Freedom of Information Protection of Privacy Act and will be used to administer program registrations, membership management, and facility bookings for Community Services. Aggregate data will be used for program planning and evaluation. Questions about the collection of personal information should be directed to the Community Services FOIP Coordinator at

Refund Policy

DALLUYON Basketball will only refund payments in full for the following reasons: program cancellation by DALLUYON Basketball and medical issues (must provide a doctor’s note).
